Job Description
- Monitor the rational use of drugs and provide drug services to patients, patient families and other health workers who need them to ensure the management and use of drugs in accordance with applicable standards, policies, laws and regulations
- Develop a drug formulary / list of drugs used in the hospital in collaboration with doctors and policy holders in the field of drugs in the hospital, including analyzing new drugs
- Carrying out prevention and monitoring activities as well as evaluating the prescription and use of medicines by patients, including making regular visits to patient wards, conducting drug assessments (prospective and retrospective), and carrying out MESO to realize healthy behavior of rational drug use
- Designing, implementing and monitoring drug therapy which includes identifying actual and potential Drug Related Problems (DRP) regarding the use of drugs given by doctors to patients.
- Providing consultations to doctors regarding drug interactions, side effects, and dosage as well as to other professional professionals regarding drug therapy to support the treatment given to patients
- Provide education and training to Pharmacists, Assistant Pharmacists and other related health workers regarding drugs to ensure that workers' competence and knowledge can meet the needs of patients and hospitals
- Providing education and information to patients, patient families and visitors who need it through various means as a form of health education
- Collect required data and / or create reports related to operational activities to be used as analysis material by superiors or by other related parties
- Implement procedures and practices for quality and safety services, including attending training, collecting data, documenting and / or making reports needed to support the achievement of hospital wide indicators and related department / unit targets
- Master Degree from Clinical Pharmacy
- Clinical Pharmacy
- Preferably have at least 1 year experience as a Clinical Pharmacist in a hospital
- In-depth knowledge of pharmacology, therapeutic drug monitoring, and patient care principles.
- Excellent communication skills for interacting with patients, families, and healthcare professionals, and for providing clear explanations and recommendations.
- Strong analytical skills for evaluating medication therapy and identifying potential issues or interactions.
Qualifications
